Checklist for your first appointment

Please arrive at least 20 minutes prior to your first appointment and bring the following items with you:

• Your current insurance card

• Applicable insurance co-pay/deductible: Payments depend are your insurance plan and are collected when you check in.

• A valid photo ID

• A list of all medications you are currently taking

Checklist for Your Subsequent Appointments

Please arrive at least 15 minutes prior to your appointment.
The following should be brought with you to every appointment:

• Your current insurance card

• Your applicable insurance copay/deductible

Your appointment is reserved especially for you, so if you need to cancel or reschedule please notify our office at least 24 hours prior to your scheduled appointment.
